Transgender refers to groups, individuals or behaviors which do not match that of the assigned sex.
For example, if a man (Ted) is born but thinks of himself as a woman trapped in a man’s body, he is transgendered. There are several different ways for someone like Ted to deal with their transgendered identity. They can remain as the sex they were born as, and date members of the same sex. So, Ted would remain a man, but have gay sex, with himself being the female, or receiver, in the dynamic.
Or, Ted could have sexual reassignment surgery, to literally make him a woman. In such operations, his penis and testicles would be removed, and a pussy installed. Ouch!
